A Good Analogy of Our System of Management

I have been in the workforce for about 3 decades.  So far, I think I have seen it all.  I tend to be a goal oriented person who is geared towards solving problems.  I take things for as they are.  I work with what I have and make the best of it. 

Recently, I found this article about an analogy about the American style of management vs the Japanese style of management.  I know from personal experience, that too often, no matter how obvious the problem is, the obvious, plain simple solution is outright ignored.  Catastrophic failure is inevitable and is considered acceptable to management.  Here is that article.
